Teacher evaluation forms and school transcripts/records are due January 29, 2025.

- Math Teacher Evaluation
- Principal/Counselor Evaluation
- School Records - report cards from 7th grade and the first semester/trimester of 8th grade & all standardized testing
Once available, you will request these documents in your admissions account. You must complete the parent/guardian release form in order to request school records.
Once the teacher/principal/registrar has completed and submitted the document, your applicant checklist will be updated to show that the form was received. If you are applying to more than one Catholic high school, you should link your recommendations from one school to the other school(s).
