Application Forms | Recommendations and Records Requests

Applicants must submit recommendation forms from a current math teacher, English teacher, and principal or counselor. Your current school must also submit transcripts from last year and the first grading period of this year as well as standardized test scores.

    • Log in to your admissions account.
    • Under your admissions checklist, click on a teacher recommendation form. You will need to input the email address of the teacher who will complete the form.
    • When you submit the form, the online system will email the request to the teacher.
    • Repeat this process for each of the recommendation forms and also the Transcript and Standardized Testing Request Form.
Transcripts and recommendations are due to La Salle by Feb. 3. You may check on the status of your recommendation requests in your admissions account. Recommendations are confidential and will not be shared with applicants.

Families are welcome to give the paper forms to teachers or school. Recommendation forms must be signed by a parent in order to give appropriate consent. Schools will send the completed recommendation forms directly to La Salle.
