You will complete four online forms and can also check on your child's physical exam & medical history forms to make sure they are current and last through the upcoming school year. Remember the physical exam and medical history forms last two years from the date of the visit. Hard copies are kept on file in the athletic office. Please mail or drop off recently completed physical exam and medical history forms Monday through Thursday during summer school office hours (8:30am - 3:30pm).

** Parents new to La Salle or who did not attend a positive coaching alliance session during the 2018-2019 school year are required to attend. A3 - Advanced Athlete Academy Summer Performance Training Program
Sessions occur Monday through Thursday from June 17 to August 15. Male athletes train from 7:30 to 9:00 am and female athletes go from 9:30 to 11:00 am. This is an excellent opportunity to work on developing strength, agility, quickness, overall athleticism, and prevent injury. Cost is $100 per athlete with checks payable to A3. Registration forms and checks must be turned into coach Aaron Hazel, Mrs. Stroud in the athletic office or on June 17th to the A3 staff.
