ATHLETIC CLEARANCE

To ensure your child is officially registered, cleared and eligible to participate for the 2025-2026 school year, please complete the athletic clearance process for your student to be cleared for participation.

Turn in a Physical Exam Form - Medical providers conducting physicals on or after May 1, 2024 must use the new form dated April 2023. Physical exam forms may be delivered in person to school, via mail to the athletic office, or uploaded using Final Forms.

FINAL FORMS/LA SALLE PREP ATHLETIC REGISTRATION WEBSITE

Required of ALL students participating in athletics

Every athlete and at least one parent must visit La Salle's online athletic clearance website and complete the online required forms. These only need to be completed once per year with the exception of the sport selection form, which must be done prior to each new sport season.

You will need your Student ID to sign in. Please call the front office (503-659-4155) and ask for the Registrar Office if you did not receive an email with your Student ID on it.

Physical Exam & Medical History Forms (REQUIRED)

We highly recommend turning in a physical exam form as early as possible to have it added to our records (by mail or bring to front office)

The Oregon Department of Education and Oregon School Activities Association (OSAA) mandate students receive a physical exam every two years in order to participate in athletics during high school. They remain valid for two years from the date of the examination. Most student-athletes have a second physical during the summer prior to their junior year if they received an exam prior to the start of their freshman year. If you wish to learn if your physical exam is still current, you may inquire by emailing athletics@lsprep.org.
